Agreement

I am taking part in the Lyme case study and commit to a course of five Biomagnetic Pair Therapy treatments at Soul-Full Sanctuary, Shrewsbury, spaced as advised by the practitioner.

I agree the discounted costs of the course: treatment 1 free (normally £80), then £40, £50, £60 and £70 — a total of £220, payable per session. Follow up sessions after the course are £75.

I agree to complete a pre-session form before treatment and a post-session form after each treatment, describing my symptoms and experience as honestly as I can.

I understand my experience may be shared as part of the case study in anonymised form, and that this case study reflects one person's experience and should not be interpreted as evidence that the treatment can cure or treat Lyme disease.

I understand Biomagnetic Pair Therapy is a complementary therapy, is not a substitute for medical advice, and that I should not stop any prescribed medication without consulting my doctor.
